Governor James McGreevey has signed Executive Order #18, to combat Identification Fraud. This executive order is designed to reduce instances of forgery and fraud in the issuance of all vital statistic records, including birth, death and marriage certificates.

The Executive Order requires that the Wantage Registrar of Vital Statistics only issue a certified copy of a vital record to persons who are the subject of the vital record, or are the subject's parent, legal guardian, spouse, child, grandchild or sibling, if of legal age. Certified copies can also be obtained in certain other limited circumstances such as upon court order.

There are Application forms available for obtaining a copy of a Birth, Death or Marriage Certificate. All of the information requested on the form must be provided, as well as the required proof of identify, and the appropriate fee, before a certificate will be issued.
